- Form: Personalized License Plate Relinquish Rights Form (revised 9/2023). It names both you and the person getting the rights.
- Steps:
- Complete and sign the form.
- Mail it to DFA Revenue Division, Personalized Plate Unit, P.O. Box 1272, Room 1130, Little Rock, AR 72203, or fax it to (501) 537-5716.
- The buyer then orders the plate.
